Include a short description to introduce your documents to recipients via the Welcome Forms web form. The form allows recipients to get acquainted with the documents (it can be a part of the survey, for example) and your company. The recipients can open the Welcome form, review the content inside it, and then successfully move to the next document.

You can upload an image or video to your form by clicking on the corresponding field. This will open the Properties settings on the right:
The Off tab leaves the form without any image or video.
In the Image tab, you can upload the desired image:
Once uploaded, you will be able to adjust the size and alignment of the image:
The image is located at the bottom of the form:
In the Video tab, you can add the link to the video you want to appear in your form:
